Going out on a limb here...looking for a line chock for our 1989 48ft CPMY. Pic is of the port side, we need the starboard side (not sure it matters). Overall length is ~8 3/8 in, screw holes are 6 1/2 in, on center.

That should be an easy find in most any marine store or catalog.

I would call that a fairlead. Might be different over there, or it might help a shop assistant (or a search)find one.
 
That's technically an open chock. Many mistakenly call the closed chocks in their bulwarks hawse holes.
 
I replaced my 1985 era chocks by perusing E-Bay. Perfect replacement and they were new old stock. Takes a bit of searching, but it is out there.
 
I bought some large chocks from Hamilton Marine for our last boat.
